Clinical Characteristics of Spontaneous Intracranial Hypotension and Epidural Blood Patch

Abstract
Spontaneous intracranial hypotension (SIH) is a syndrome characterized by postural headache without any preceding events such as major trauma, spinal anesthesia or other medical illness.
The most common cause of SIH is leakage of the spinal cerebrospinal fluid (CSF). An autologous epidural blood patch (EBP) is an effective treatment for managing postural headache that persist despite conservative treatments. We report a case of SIH which resolved after 3 times epidural blood patches in 44-years-old woman. The first EBP which was done at L2-L3 level, improved the patient¡¯s headache, but the headache recurred in one month. On her computerized tomographic myelography (CT-myelography), we found nodular calcification of ligamentum flavum at 10th thoracic spine level with CSF leakage at the whole thoracic level. Two more applications of EBPs were done at thoracic level and her headache was resolved gradually without any serious complications.
